THE FIRST SEMEN ANALYSIS

Time or Ejaculations?

A study by Barone et al, (J Urol. 2003 Sep;170(3):892-6) reported on the results of a multi-center study reviewing the results of vasectomy using standard ligation and excision. While it has been shown that more effective methods exist for performing vasectomy, some interesting observations can be made from this study. At 12-weeks 60% of men were azoospermic, but at 20 ejaculations only 27.9% of men were azoospermic. By 24 weeks only 81.5% achieved azoospermia. The authors suggest that both a stipulated period of time as well as a minimum number of ejaculations is necessary before semen analysis should be performed. From these results it can be observed that azoospermia may be an unrealistic measurement upon which to base success. The study also showed that men continue trending toward azoospermia over time (at least up to 24-weeks) and that number of ejaculations alone is not an appropriate basis for determining the timing of semen analysis.
